After cataract surgery, a gradual decrease in vision may occasionally occur again – this is known as posterior capsule opacification (PCO). It refers to a clouding of the eye’s natural lens capsule in which the artificial lens is placed. This thickened capsule causes vision to become cloudy but can be effectively treated using a YAG laser. In a procedure called YAG laser capsulotomy, the eye doctor uses the laser to painlessly dissolve the thickened part of the capsule. In most cases, this restores vision completely.
Causes & Symptoms of Posterior Capsule Opacification
Cataracts are commonly treated today by replacing the clouded natural lens with an artificial intraocular lens (IOL) that is placed into the eye’s own lens capsule. After surgery, some patients may experience a thickening of the capsule, which manifests as clouding. Vision may deteriorate, and eyesight may appear hazy or blurry.
PCO can occur when certain cells from the natural lens capsule begin to multiply and form fibrous structures along the visual axis. Additionally, residual lens epithelial cells may remain in the capsule after the artificial lens is implanted. If these cells begin to proliferate, they can also cause clouding.
It is important to note that posterior capsule opacification is not a complication of cataract surgery and does not reflect the quality of the procedure. Rather, it is a natural healing response of the body to the implanted lens. If the body’s healing response is particularly strong—as is often the case in younger individuals—PCO may develop within the first few years following cataract surgery.
Procedure of Posterior Capsule Opacification (PCO) Treatment with the YAG Laser
YAG laser treatment for posterior capsule opacification (secondary cataract) is a quick and painless procedure that only takes a few minutes. Before the laser treatment, your pupils will be dilated with eye drops, which means you should not drive yourself afterwards.
During the YAG capsulotomy, the ophthalmologist uses a slit lamp to which the YAG laser is attached. The doctor precisely directs the laser beam at the thickened, clouded lens capsule, creating an opening in its center.
After the laser treatment, you can return home immediately. Once the effects of the pupil-dilating drops wear off—usually after a few hours—your vision should already be clearer and continue to improve over the following days.
Frequently Asked Questions about Posterior Capsule Opacification (PCO) Treatment
Who is suitable for YAG laser treatment?
The YAG laser treatment for posterior capsule opacification (PCO) is a non-invasive procedure and is therefore generally suitable for all patients who develop PCO.
If vision deteriorates again after cataract surgery, it should be checked whether PCO has occurred. This can then be painlessly treated with a YAG laser capsulotomy.
What are the benefits of YAG laser treatment?
YAG laser treatment for posterior capsule opacification is safe, painless, and takes only a few minutes. In the vast majority of cases, the recurrence of clouding can be permanently eliminated. The results are visible almost immediately.
Is treatment with the YAG laser painful?
Before your YAG laser treatment in Linz and Vienna, your eyes will be pre-treated with pupil-dilating eye drops. The actual posterior capsule opacification (PCO) treatment is contact-free and therefore completely painless.
How long does the aftercare treatment for posterior capsule opacification take?
The YAG capsulotomy only takes a few minutes and is performed on an outpatient basis. Afterwards, you can leave the practice immediately. The only restriction is that, due to the dilated pupils, you are temporarily not allowed to participate in road traffic.
When will the result of the treatment become noticeable?
In the posterior capsulotomy treatment with the YAG laser, the thickened lens capsule is immediately opened. Therefore, an improvement in vision usually occurs right away. Once the effect of the eye drops has worn off, you should be able to see clearly again.
What are the success rates of a YAG laser capsulotomy?
The success rates of posterior capsular opacification treatment with the YAG laser are very high. In most cases, vision can be fully restored. However, if the eye is already damaged, an optimal result may not be achieved.
What should you keep in mind after a YAG laser capsulotomy?
After your posterior capsule opacification treatment in Linz and Vienna, you will temporarily not be fit to drive. Therefore, you should be accompanied to your appointment or arranged to be picked up afterward.
To prevent inflammation, your ophthalmologist will prescribe anti-inflammatory eye drops, which you should use several times daily for about one week.
What are the risks associated with posterior capsulotomy treatment?
The YAG laser capsulotomy is a low-risk procedure when performed by an experienced ophthalmologist. Complications are very rare with this non-invasive treatment.
Intraocular pressure may temporarily increase after the YAG laser treatment. To prevent complications, regular follow-up examinations are scheduled.
Occasionally, residual posterior capsule opacification particles remain in the vitreous humor and are perceived as small black floating spots. In rare cases, fluid accumulation can occur in the central retina area. This so-called cystoid macular edema can usually be treated effectively with medication.
Thanks to modern treatment methods, the risk of retinal detachment following YAG capsulotomy is very low today. Retinal detachment can generally be successfully treated surgically.
